Social Worker

The Social Worker position is a member of the hospice interdisciplinary team. As strong advocates for patients and families, our social workers assess needs and plan interventions to help patients and their families: navigate planning for end-of-life care; understand their plan of care and be vocal about their needs; manage the stresses of debilitating physical illnesses, including emotional, familial and financial; overcome crisis situations; and connect to other community agency support services. Position will also assist in the grief and healing process, including facilitating community support groups, and providing support for individuals, families, children and groups as needed. Rewards include making strong connections with people, learning and celebrating life stories, and having a positive impact on patients and their caregivers.

Full-time

Pay range: $53,000-$63,000
