Admission RN

As a clinician providing the first visit with patients and caregivers, the Admissions RN makes a lasting positive impact, brightening their days, symbolizing relief and instilling confidence in the care and support Helios Care offers. As a member of the Access Team, the Admission RN works with clients, case managers, doctors and or primary RNs to facilitate the start of services, and provides education regarding Hospice and Palliative Care services, philosophies and eligibility. The Admissions RN completes the initial comprehensive assessment of patient and family to determine needs, initiate the Plan of Care, and provides exceptional nursing care to promote comfort of the patient and their caregivers.

MINIMUM REQUIREMENTS
▪ Graduate of an accredited school of nursing.
▪ One to two years of recent acute care experience in an institutional setting.
▪ Ability to represent the organization professionally at all times.
▪ Ability to be flexible and adaptable to changing workload hours.
▪ Current professional licensure in New York State, and valid driver’s license with driving
record which meets standards outlined in Policy: Qualifications of Hospice Personnel.
▪ Bachelor’s degree, with one year of home health care experience. Registered nurses with
less than a Bachelors degree will be considered based on previous experience.
▪ Management experience is not required but supervisory skills preferred
▪ Prolonged or considerable walking or standing. Able to lift, position and/or transfer
patients. Able to lift supplies and equipment. Considerable reaching stooping, bending,
kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
▪ Excellent observation, verbal and written communication skills, problem solving skills,
customer service skills, and

$77,500.00 to $80,500.00 per year
