The principle responsibility of this role is to serve as the main Helios Care liaison to nursing home facilities in the service area. This position is responsible for offering information and
support to patients in skilled nursing facilities, including the needs for hospice or palliative care.
This person is responsible for providing Hospice RN Case Management, symptom management and discharge planning for patients in local skilled nursing facilities. This role will also perform hospice admissions and PRN visits to current hospice patients in the home setting as needed. Provide Hospice RN Case Management for a routine home care hospice patient as needed.
Full time
Pay Range: $77,500 -$83,500
Requirements:

Bachelor of Science in Nursing (BSN) required; candidates with an Associate Degree in Nursing will be considered based on relevant clinical experience.

Minimum of one (1) year of home health care experience preferred.

Current and unrestricted Registered Nurse (RN) licensure in New York State required.

One to two (1–2) years of recent acute care experience in an institutional healthcare setting preferred.
