Helios Care is proud to announce it has been awarded a significant grant, $195,000, from the Mother Cabrini Health Foundation to support its Choices program.
The Choices Program, which supports patients in Delaware, Otsego, and Schoharie counties, was created to bring in-home palliative care services to older adults who are facing a serious illness. Palliative care is specialized care with a focus on symptom relief and quality of life.
With New York’s older adult population growing rapidly and access to care being limited, programs like Choices play a crucial role in helping individuals age safely at home while maintaining a higher quality of life with less pain, reduced isolation, and greater support.
Thanks to the Mother Cabrini Health Foundation, Helios Care can serve these patients at no cost to them and their families.
“Choices has evolved thanks to the continuing relationship between Helios Care and the Mother Cabrini Health Foundation,” said Kathryn Dailey, Director of Development for Helios Care. “With this critical grant, Choices brings in-home person-centered custom care to patients, addressing their emotional, social, and physical needs. Helping them to feel better with this extra layer of support along with their current and ongoing treatments.”
The Mother Cabrini Health Foundation is a private, nonprofit organization whose mission is to improve the health and wellbeing of New Yorkers, bolster the health outcomes of vulnerable communities, eliminate barriers to care, and bridge gaps in health services. The Foundation awarded more than 399 grants to community-based organizations in New York State in their 2025 for 2026 award cycle, totaling $142 million to help enhance patient health outcomes in the state.
